  • Delivery of the thump: The rescuer delivers a forceful blow using the heel of their hand or a closed fist with significant downward pressure.
  • Monitoring response: After delivering the thump, the rescuer should assess for any signs of return of spontaneous circulation (ROSC). If ROSC occurs, immediate cardiopulmonary resuscitation (CPR) should be initiated.

Indications for Precordial Thump:

  • Sudden witnessed collapse with monitored ventricular tachycardia (VT) or ventricular fibrillation (VF).

Contraindications of Precordial Thump:

The precordial thump is contraindicated in certain scenarios:

  • Pediatric patients: It is not recommended for infants and children due to the risk of causing harm.
  • Asystole: The precordial thump is not effective in treating asystole (flatline) rhythms.
  • Availability of defibrillation: If defibrillation equipment is immediately available, it should be the primary intervention rather than relying on the precordial thump.

Limitations and Considerations:

While the precordial thump can be a potentially life-saving intervention in specific circumstances, it has limitations that should be recognized:

  • Decreased success rate: The success rate of precordial thump in converting VT/VF to a normal rhythm is relatively low compared to defibrillation.
  • Time-sensitive intervention: The precordial thump should only be attempted for a brief period until defibrillation or other advanced cardiac life support measures are available.
  • Need for proper training: Healthcare providers should receive adequate training and education in the technique of precordial thump to ensure its correct application.
