Introduction:

Orchidopexy, also known as orchiopexy, is a surgical procedure performed to correct undescended testicles in children. This condition, known as cryptorchidism, occurs when one or both testicles fail to descend into the scrotum during fetal development or early infancy. Orchidopexy is a crucial intervention to ensure proper testicular development and reduce the risk of complications later in life.

Understanding Cryptorchidism:

Cryptorchidism is a common congenital condition, occurring in about 1-4% of full-term male infants and 30% of premature male infants. While in utero, the testicles develop within the abdomen and typically descend into the scrotum by the time of birth or during the first few months of life. However, when one or both testicles remain undescended, it can lead to various concerns, including fertility issues and an increased risk of testicular cancer.

Indications for Orchidopexy:

Orchidopexy is typically recommended for several reasons:

  • Infertility Prevention: Undescended testicles can lead to impaired sperm production and fertility issues later in life. Orchidopexy helps mitigate this risk by facilitating proper testicular development.
  • Reducing Cancer Risk: Men with a history of cryptorchidism have a higher risk of testicular cancer. Orchidopexy is believed to reduce this risk, although regular self-examinations are still essential.
  • Psychosocial Benefits: Correcting undescended testicles can also have psychological benefits, as it avoids potential emotional distress or self-esteem issues that may arise due to the condition.

The Orchidopexy Procedure:

The orchidopexy procedure involves surgically moving the undescended testicle(s) from their abnormal location into the scrotum. Here are the key steps:

  • Anesthesia: The child is placed under general anesthesia to ensure they remain pain-free and asleep throughout the procedure.
  • Incision: The surgeon makes a small incision in the groin area or the scrotum, depending on the testicle's location.
  • Locating the Testicle: The surgeon carefully locates the undescended testicle, which may be inside the abdomen or along the inguinal canal.
  • Detaching Adhesions: In some cases, adhesions or abnormal connections may need to be detached to free the testicle.
  • Creating a Scrotal Pouch: A pouch is created in the scrotum to accommodate the testicle.
  • Securing the Testicle: The testicle is gently moved into the scrotal pouch and secured in place.
  • Closing Incisions: The incisions are closed with dissolvable stitches or surgical glue.

Postoperative Care:

After orchidopexy, careful postoperative care is essential. This may include:

  • Pain Management: Managing post-surgery discomfort with pain relievers as prescribed by the surgeon.
  • Monitoring: Regular follow-up appointments to ensure proper healing and testicular function.
  • Activity Restrictions: Limiting strenuous activities for a few weeks to aid the healing process.

Conclusion:

Orchidopexy is a critical surgical procedure to correct undescended testicles in children, addressing potential complications such as infertility and an increased risk of testicular cancer. Timely diagnosis and intervention, often before the age of two, can significantly improve outcomes.
